The Treponema Pallidum Haemagglutination Assay, (TPHA) is used for either confirmation of syphilis or as a routine second test. The reagent system is red blood cells coated with antigen of Treponema pallidum. Often avian cells are used as these are nucleated and therefore heavier, producing a faster reaction time.

In this test the patient's sample is incubated at room temperature with the reagent in a 96 well microtitre plate.
With avian cells incubation time is 30-45 minutes and a positive result is indicated by a ‘carpet’ of red cells across the well. A negative reaction is indicated by a button or ring of red cells across the bottom of the well. It is important to note that incubation must take place on a vibration-free bench and away from direct sunlight or other heat source or any other source of variable temperature across the microtitre plate such as draughts.

حتى لو عملنا TPHA لانعترف به حتى لو انه specific test
لابد من عمل trep IgM
لماذا لان فيه نوع من treponema غير treponema pallidum ممكن تعطي false positive
