    ]الألفا فيتو بروتين(Alpha Fetoprotein) مادة بروتينيه تصنع في الكبد و لكن لا يعرف وظيفته الأساسية.و لقد وجد الأطباء أن قياس هذا البروتين في دم الأم الحامل له فوائد تشخيصية لبعض الأمراض العيوب الخلقية.و الجنين ينتج كميات متفاوتة من هذا البروتين و يمر عبر الحبل السري و المشيمة إلى دم الأم. و لذلك فعند ارتفاع نسبته عند الجنين فان تلك الزيادة أيضا تظهر على شكل ارتفاع في مستوى البروتين في دم الأم.و لقد و جد أن ارتفاعه بشكل ملحوظ قد يعني أن هناك عيبا خلقيا في العمود الفقري للجنين أو بما يعرف بالظهر المشقوق كما انه أيضا يرتفع في حالات و جود ثقب أو فتحة في جدار البطن كما هو الحال في حالة و جود فتق في السره من النوع الذي يكون مصحوب بخروج الأمعاء.كما انه يرتفع في حالة و جود رشح للبروتينات من الكلية او ما يسمى بمرض نفروتك(Finish Type Nephrotic syndrome).
    و من المعلوم أن مستوى الألفا فيتو بروتين يتفاوت على حسب عمر الجنين و لذلك فان لكل أسبوع من الحمل مستوى محدد إذا تجاوزه اعتبر مرتفعا.و لذلك فان أو عمل يقوم به الطبيب عند ارتفاع هذا البروتين هو التأكد من عمر الجنين عند اخذ العينية.كما أن مستوى هذا البروتين يرتفع عند و جود توائم, كما أن مستواه يتفاوت إلى حد بسيط حسب و زن الحامل و عمرها و حسب أصولها العرقية.في العادة يجرى هوا التحليل في الأسبوع السادس عشر من الحمل لكل النساء و يمكن إجراءه بعد ذلك التاريخ.
    كما أن انخفاض هذا البروتين قد يزيد من احتمال أن يكون الجنين لديه مشكلة في الكروموسومات و بالتحديد مرض متلازمة داون.
    إن من المهم معرفة أن تحليل مستوى الألفا فيتو بروتين لا يُعتمد عليه في التشخيص. هو فقد يحدد نسبة احتمال الإصابة.فيقول مثلا أن احتمال الإصابة بمرض ما أعلى من المتوقع أو مساوي للمتوقع.و لذلك فإذا كان التحليل غير طبيعي فانه يلزم الطبيب إجراء فحوصات إضافية للتأكد من سلامة الجنين.فمثلا إذا كان مستوى البروتين عالي فانه يقوم –بعد التأكد من عمر الجنين وقت إجراء الفحص-بالتأكد من عدم و جود توائم و عدم و جود عيب خلقي في العمود الفقري(الظهر المشقوق أو حالة غياب المخ ) أو في جدار البطن و يتم ذلك بالأشعة الصوتية و عند الجمع بين هذا التحليل و الأشعة الصوتية فانه تستطيع أن تكشف الظهر المشقوق بنسبة 95% و تكشف حالة غياب المخ بنسبة 100%. و عند إجراء تحليل الألفا فيتو بروتين فقط فإنها تكشف عن 85% من الحالات فقط.
    كذلك الحال عند انخفاض مستوى الألفا فيتو بروتين عن المعدل الطبيعي فانه يجب التأكد من أن الجنين ليس لدية علامات أو أعراض لمتلازمة داون عن طريق إجراء أشعة صوتية للجنين،و قد ينصح الطبيب الأم الحامل بإجراء اختبار للسائل الامينوسي للكشف على كروموسومات الجنين للتأكد بشكل أدق من عدم أو إصابة الجنين بمتلازمة داون أو إدوارد.
    لقد سبب هذا التحليل الكثير من القلق للأمهات الحوامل عند ما يكون نتائج التحليل غير متوقعه.و قد يكون هذا القلق مبالغ فيه إلى حد بعيد خاصة إذا علمنا أن الكثير من كان لديهم قراءات غير متوقعة كان أطفالهم سليمين. أن الفائدة الحقيقية من إجراء هذا الفحص هو للتقليل من عدد النساء التي يجب إجراء فحص لسائل الامينوسي لهم للكشف عن متلازمة داون. فمن غير الممكن إجراء فحص للسائل الامينوسي لجميع النساء. لذلك اختير تحليل الألفا فيتو بروتين لكي يساعد في توجيه انتباه الأطباء إلى بعض النساء اللاتي يلزم إجراء الفحص الأامينوسي لهن.أن عدم إجراء اختبار السائل الامينوسي لكل النساء هو لارتفاع تكلفة إجراءه،إضافة إلى انه قد يسبب الإجهاض بنسبة 0.5% من النساء و هذا الرقم قد يكون منخفض و لكنه بلاشك اقل من نسبة احتمال أن يكون الجنين مصاب بمتلازمة داون هي نسبة تصل إلى 1% عند بلوغ المرأة سن الخامس و الثلاثون العمر.و لذلك فالأطباء ينصحون جميع النساء التي أعمارهن فوق 35 سنة أن يقوموا بإجراء فحص للسائل الامينوسي مباشرة و ذلك للتأكد من أن الجنين ليس مصاب بمتلازمة داون علما أن حتى لو اكتشف الأمر فان الحل الوحيد في هذه الحالة هي الإجهاض إذا كان ذلك يجوز شرعا و برغبة من الوالدين
    How is it used?
    The triple or quad screen is used as a screening test in the second trimester of pregnancy. It is ordered to help evaluate the risk that a fetus has certain abnormalities, including Down syndrome (trisomy 21), Edwards syndrome (trisomy 18), and neural tube defects such as spina bifida or a condition called anencephaly.
    These tests have been established as a triple or quad screen because their power lies in their use together. A mathematical calculation involving the levels of these 3 or 4 substances (AFP, hCG, unconjugated estriol, and, sometimes, inhibin A) and considerations of maternal age, family history, weight, race, and diabetic status are used to determine a numeric risk for Down syndrome and for a few other chromosomal abnormalities such as Edwards syndrome (trisomy 18) in the fetus. This risk is compared with an established cut-off. If the risk is higher than the cut-off value, then it is considered a positive screening test or increased risk exists for carrying a fetus with one of the discussed abnormalities.
    An AFP test may be performed by itself and not as part of a triple or quad screen, especially when First Trimester Screening has already been used to assess the risk for chromosomal abnormalities. The AFP is used to help determine the risk of neural tube defects.
    When is it ordered?
    The test is usually ordered between the 15th and 21st weeks of pregnancy.
    What does the test result mean?
    The interpretation of a test result should be provided by a genetic counselor or clinician who can explain the meaning of the results and offer choices about follow-up. It is important to remember that screening tests are not diagnostic of a fetal abnormality; they indicate a normal or increased risk. Of all women who have positive triple or quad screening results, only a very small number of them have babies who actually have a neural tube defect or chromosomal abnormality.
    In pregnancies where the fetus is carrying the chromosomal defect that results in Down syndrome (trisomy 21), the levels of AFP and unconjugated estriol tend to be low and hCG and inhibin A levels high.
    In pregnancies where the fetus has Edwards syndrome (trisomy 18), unconjugated estriol and hCG levels are low and AFP levels can be variable.
    A baby with an open neural tube defect has an opening in its spine, head, or abdominal wall that allows higher-than-usual amounts of AFP to pass into the mother's blood. The other markers are not used in the evaluation of risk for carrying a fetus with a neural tube defect.
    If a screen is positive, tests that are more definitive are needed to determine and confirm a diagnosis. These include high-resolution ultrasound and perhaps amniocentesis followed by chromosome analysis. These follow-up tests are used to help women and their doctors make decisions about the management of their pregnancies.
    The triple screen can detect about 70% of fetuses affected by Down syndrome while the quad screen can detect about 80%. Either screen can detect about 75-80% of those affected with spina bifida and 95% cases of anencephaly.
    Not all fetal abnormalities will give positive test results.
    The test result is very dependent on accurate determination of the gestational age of the fetus. If the gestational age of the fetus has not been accurately determined, the results may be either falsely high or low.
    In multiple gestation pregnancies, calculation of the risk of Down syndrome or Edwards syndrome is difficult. For twin pregnancies, a "pseudorisk" can be calculated comparing results to normal results in other twin pregnancies. For higher gestation pregnancies, risk cannot be calculated from these tests.
    Evaluation of the risk of open neural tube defects in twin pregnancies can be determined, although it is not as effective as in singleton pregnancies.
    If you have had a First Trimester Screen, then a triple/quad screen (second trimester screening) is not performed because the risks for Down syndrome and Edwards syndrome have already been assessed. However, if you and your doctor wish to use the results of both first and second trimester screening to assess the risk of chromosome abnormalities, then integrated or sequential screening may be employed.     Triple test contain:
    • Human chorionic gonadotropin (hCG) is a hormone produced by the placenta. Levels in maternal blood rise for the first trimester of pregnancy and then decrease during the remainder of the pregnancy.
    • Unconjugated estriol (uE3) is a form of estrogen that is produced by the fetus through metabolism. This process involves the liver, adrenals, and the placenta. Some of the unconjugated estriol crosses the placenta and can be measured in the mother's blood. Levels rise around the 8th week and continue to increase until shortly before delivery.
    • Inhibin A is a hormone produced by the placenta. Inhibin is a dimer (has 2 parts) and is sometimes referred to as DIA or dimeric inhibin A. Levels in maternal blood decrease slightly from 14 to 17 weeks gestation and then rise again.
    Results of each of these tests are considered together to determine the risk that the fetus has a chromosomal defect or an open neural tube defect.

                        القيمة الطبيعية لهذا الفحص عند الاشخاص الأصحاء والنساء غير الحوامل
                        تكون نسبة قليلة جدا ..
                        ترتفع في حالة أمراض الكبد المزمنة كتليف الكبد والتهاب الكبد الوبائي وسرطانات الكبد والخصية والمبايض بشكل واضح

                            To help diagnose and monitor therapy for certain cancers of the liver, testes, or ovaries
                            This test measures alpha-fetoprotein (AFP) in the blood. AFP is a protein produced primarily by fetal liver and the portion of a developing embryo that is similar to the yolk cavity in bird eggs (yolk sac tissues). AFP concentrations are typically elevated when a baby is born and then decline rapidly. In healthy children and non-pregnant adults, AFP is normally only detectable at very low levels.
                            Liver damage and certain cancers can increase AFP concentrations significantly. AFP is produced whenever liver cells are regenerating. With chronic liver diseases, such as hepatitis and cirrhosis, AFP may be chronically elevated. Very high concentrations of AFP may be produced by certain tumors. This characteristic makes the AFP test useful as a tumor marker. Increased amounts of AFP are found in many people with a type of liver cancer called hepatocellular carcinoma. They are also found in some people with cancers of the testes and ovaries.
                            AFP exists in several different variants. Traditionally, when a doctor orders an AFP test, he is ordering a total AFP, one that measures all of the AFP variants together. One of the variants is called L3 because of its ability, in the laboratory, to bind to a particular protein called Lens culinaris agglutinin. The AFP-L3% test is a new test that compares the amount of total AFP to the amount of AFP-L3. An increase in the percentage of L3 to total AFP is associated with increased risk of developing hepatocellular carcinoma in the near future and of having a poorer prognosis, as the L3-related cancers tend to be more aggressive
                            Increased AFP levels may indicate the presence of cancer, most commonly liver cancer, cancer of the ovary, and germ cell tumor of the testes. However, not every liver, ovary, or testes cancer will produce significant quantities of AFP. Elevated levels may be sometimes seen with other cancers such as stomach, colon, lung, breast, and lymphoma, although it is rarely ordered to evaluate these conditions. Other diseases such as cirrhosis and hepatitis can also cause increased levels.
                            When AFP is used as a monitoring tool, decreasing levels indicate a response to treatment. If concentrations after cancer treatment do not significantly decrease, usually to normal or near normal levels, then some of the tumor tissue may still be present. If concentrations begin to increase, then it is likely that the cancer is recurring. If AFP levels are not elevated prior to treatment, then the test will not generally be useful to monitor the effectiveness of treatment or to monitor for recurrence.
                            When the AFP concentrations of people with chronic liver disease go from moderately elevated to greatly elevated, their risk of developing liver cancer increases. When total AFP and AFP-L3% are significantly elevated, then the affected person has an increased risk of having or developing hepatocellular carcinoma in the next year or two. However, both AFP and AFP-L3% concentrations can be elevated, and fluctuate, in people with chronic hepatitis and cirrhosis. In these cases, a sharp increase in AFP is more important than the actual numerical value of the test result.
                            Not every person with increased AFP and AFP-L3% test results has cancer or will develop liver cancer. The AFP and AFP-L3% tests are not diagnostic; they are indicators. They must be used in conjunction with information from a history and physical examination as well as imaging studies to look for the development of tumors. Although these tests can provide useful information, they are not as specific or sensitive as doctors would wish. AFP can temporarily increase whenever the liver is injured and regenerating, and moderate elevations can be seen with a variety of conditions. Because of this, AFP testing can give some false positives. In addition, not every cancer will produce AFP, so a person could still have cancer even when the AFP is normal. For these reasons, the AFP test should not be used to screen the general population for cancer.
