Western Blot

        HIV Testing

        Antibody
a molecule, produced by the B-cells of the immune system, that attaches to a specific infectious invader, either a) inactivating the invader or b) flagging it for destruction by other immune cells.
        ELISA
test-enzyme-linked immunosorbent assay; a test that detects HIV antibodies.
        HIV
Human Immunodeficiency Virus; the virus that causes AIDS. "Immunodeficiency" refers to a weakened, or deficient, immune system.
        Western Blot test
a test that detects HIV antibodies.
        Background:-
The body does not naturally have HIV antibodies. Antibodies are only produced in the presence of an infection. Hence, if HIV antibodies do exist, it is a sign that HIV has invaded the body. Both the ELISA test and the Western Blot test detect HIV infection by detecting the presence of HIV antibodies.
Both of these tests rely on the fact that HIV and HIV antibodies bind together. The binding site on the surface of HIV is protein called an antigen. There is an area on the surface of HIV antibodies into which HIV antigens fit, like a key fits into a lock.
        The ELISA Test:-
Antibodies are found in serum that has been separated from the red blood cells. During an ELISA test, this serum is placed in a tiny well containing a plastic bead coated with the proteins of HIV. Among these proteins are antigens. If there are any HIV antibodies in the serum, they will bind to the HIV antigens on the bead. An indicating substance is then added which will turn yellow if any binding has taken place. This multi-layer process can be visualized as in the diagram to the left.
If the test is negative, and the patient may have been exposed to HIV, it is wise to test again in six months. The negative result may simply mean that antibodies have not yet been produced. If the test is positive, it is repeated. If the second test is positive, the Western Blot test is performed.
        The Western Blot Test:-
In this test, the proteins of HIV are laid out vertically, from largest to smallest, on a strip of film. Serum is added and any existing HIV antibodies will bind to the HIV antigens. A detection enzyme is then added, which latches on to the antibodies. Finally, a chemical is added that changes color when it comes into contact with the protein-antibody-enzyme layers. This multi-layer process is similar to that of the ELISA test. However, the final result is a unique series of shaded bands, like the example .
This picture includes a positive control, a negative control, an indeterminate Western Blot test, and a positive Western Blot test.
Interpretation of the Western Blot can be complicated, but a good rule of thumb is the "3 band rule": If three or more bands appear, there is solid evidence that antibodies to HIV have been detected. If one or two bands appear, the test is considered indeterminate. If a patient with an indeterminate test has engaged in high risk behavior, another test should be conducted in six months.
        Additional Western Blot Facts:-
There are many bands in a strong positive test because, technically, any proteins that are foreign to the body are antigens. These proteins do not necessarily have to be on the outside of the virus. As a result, there are actually several types of HIV antibodies, each targeting a different HIV protein. The more advanced an infection, the more types of HIV antibodies exist and the more bands that appear.
Some bands are darker than others because some proteins are more abundant in the virus. Hence, during the test, more antibodies stick to that portion of the strip.)
        Why have two tests?
If the Western Blot is negative, does that mean the ELISA tests were performed incorrectly? Not at all. The problem is that some proteins found in HIV are similar to proteins found in other infectious agents. As a result, just because something in the serum binds to a protein from HIV does not necessarily mean that HIV is present. For this reason, the Western Blot is more accurate than the ELISA test because it can indicate what, exactly, is binding to what. However, the ELISA test is easier and less expensive and can detect infection earlier. In combination, the two tests make a good team.

                        HIV 1 NEGATIVE
                        HIV 2 Negative
                        HIV P24 AG NEGATIVE
