International Normalized Ratio (INR)

The international normalized ratio (INR) system is viewed as the best means of standardizing measurement of prothrombin time to monitor oral anticoagulant therapy. It isn't used as a screening test for coagulopathies.

تعريف آخر

The accuracy of the Prothrombin Time (PT) is known to be very system-dependent. The World Health Organization has addressed this system variability problem by (1) the establishment of primary and secondary international reference preparations of thromboplastin and (2) the development of a statistical model for the calibration of thromboplastins to derive the International Sensitivity Index (ISI) and the INR.

The INR uses the ISI to equate all thromboplastins to the reference thromboplastin through the following equation:

INR = (patient PT/mean normal PT)

الهدف منه

Purpose

To evaluate effectiveness of oral anticoagulant therapy.

Reference values

Normal INR for those receiving warfarin therapy is 2.0 to 3.0. For those with mechanical prosthetic heart valves, an INR of2.5 to 3.5 is suggested

أهم مافي الامر بالنسبة للعملي هذا القانون ..

INR = result of PT Patient / result of PT Control * ISI

ISI = International Sensitivity Index
وتعبر عن مدى نشاط المحلول المستخدم

لكن السؤال ماهي قيمة الـ ISI .. اعتقد انها قمية ثابتة .. !! ولكن كم هي ؟؟

و بالنسبة للـــ ISI بحث عنها في الانترنت و رجعت للــprosdure manuals اللي عندي التابعة لجهاز الــ Dade Behring BCS

و لقيت ان هذي القيمة provided by the manufactor ....

the slope of the line of best fit relating the log prothrombin time obtained with a standard reagent to the log prothrombin time obtained with the working reagent for both normals and patients who receive stable oral anticoagulant therapy; the standard reagents used for this value assignment are reference preparations calibrated against the World Health Organization standard reagent.

The ISI value indicates how the particular batch of tissue factor compares to an internationally standardized sample. The ISI is usually between 1.0 and 1.4.

يطلب هدا الفحص غالبا في تتبع الحالات المعالجه بال وارفيرين والوارفيرين يستخدم لزياده ميوعه الدم كما في حالات احتشاء عضله القلب فادا كانت ال inr منخفضه يقلل الطبيب الجرعه والفحص مهم جدا بالنسبه لل PT يعمل يدويا بعد ان تعمل لك كونترول خاص بالمختبر حقك وهو لمجموعه اشخاص اصحاء لايتعاطون اي ادويه مميعه للدم وياتي مع الكت شارت يبين النورمل وفي الخانه الاخرى يكون ارقام وبعد ان تعمل الفحص وحسابه بالثواني تقراء النتيجه ويكون الinrمكتوب والله يوفقك

the ISI is important when you bring a new lot number from the company that supply you with the PT kit as for me i try to bring all the kits available in the supplying company so to astabalize the PT results in my lab also you need to check your reagent usually in fixed times by coagulation control samples and also from time to time by normal patient samples the lower the ISI the better the stability of the PT test results i would like to mention that the PT samples should be done as soon as possible not more than 4 hours or you must freeze the citrated plasma if more also before doing the test you need to centrifuge the patient sample at high speed ( 3000 RPM) for 15-20 min to get platele poor plasma ( not more than 10) you need to check the patient old results to see if its going with the dose that he talk and the most important to tell the patient never stop your medication before the test unless his doctor tell him and he must tell you that ... for any question im ready to help

انزين هل صحيح انه قيمة INR تقريبا واحد

وهل صحيح انه قيمة PT الطبيعي 30 الى 40
وPTT ايضا نفس القيمة

ال ISE هو عامل ياتي مع المحلول الخاص بقياس الPT
تحدده الشركه المصنعه للمحلول ويحدد بفاعلية تركيبة المواد بالمحلول
اما بالنسبه لل INR فهو نتيجه وقت الاختبار للعينه وتقسم على المتوسط الطبيعي وتضرب بال ISE
القيمه الطبيعيه لل PT تختلف على حسب ال LOT# للمحلول
لذلك يجب قياس المعدل الطبيعي للمحلول عند استخدامه
وتتم بجمع 20 عينه عشوائيه من اناس اصحاء وعمل التحليل لها واخذ المتوسط
لذلك تختلف القيمه الطبيعيه باختلاف ال LOT#
وبالنسبه للطبيعي تتراوح بالعاده بين 10 الى 15 ثانيه لل PT
و 20 الى 40 ثانيه للPTT
