Campylobacter and Helicobacter
General properties:
-G-ve curved bacilli (gull wing or S shape), Non spore forming, Non capsulated Motile with polar flagellum.
-Micro-aerophilic condition.
-Fastidious.
-Thermophilic.
-Oxidase +ve.
Species of Medical Importance:
-Campylobacter jejuni:
=Cause Gastroenteritis (Diarrhoea with blood and mucus).
-Campylobacter coli:
=Normal flora of the intestine
-Campylobacter fetus:
=Cause bacteremia and isolated from Blood culture.

-Helicobacter pylori:
=Cause Peptic and Duodinal ulcer.
Disease and Pathogenicity:
-C. jejuni:
*Cause diarrhoea begin with mild abdominal pain and developed after 2-3 day to severe bloody diarrhoea.
*Self limited disease.
-H. pylori:
*Low grade inflammatory process in the stomach or duodenum.
*The organism is non-invasive.
*Chronic infection may lead to Peptic cancer.
Laboratory diagnosis:
-Campylobacter spp.:
-Specimens:
-Stool.
-Rectal swab.
-Blood for culture.
-Transport media:
-Cary-Bliar. (BGS is not use due to it’s toxicity to Pathogenic campylobacter spp).
-Selective media:
-Skirrow: Blood agar supplemented with Trimethoprim, Vancomycin and Polymexin B.
-Butzler media: Blood agar supplemented with Cefoperazone, Rifampin, Colistin and Amphotricin B.
-CAMPY BAP: Brucella agar base with 10% sheep blood and Vancomycin, Trimethoprim, Polymexin B, Amphotricin B and Cephalothin.
Incubation and Cultural characters:
-Incubation:
=On Microaerophilic condition for 48 to 72 hours at 42C.
-CAMPY BAG II system.
-Evacuation replacement system (6% O2, 10% CO2, 80% N2, and the rest is H2.
-BioBag Type Cfj consist of a single plate bag and a generator ampule.
-Candle jar.
-Colonial morphology:
=Produce moist (runny looking) water droplet, circular raised and non-haemolytic
Biochemical reaction:
-Catalase +ve.
-Reduce nitrate to nitrite.
-Urease –ve.
-H2S –ve.
-Hippurate hydrolysis +ve.
-Oxidase +ve.
-Specimens:
*Gastric biopsy (by endoscope).
-Transportation:
*Stuart transport medium.
*Cysteine Brucella broth with 20% glycerol and frozen at -70°C.
-Direct Gram stain.
-Culture: on Blood agar.
-Incubation: at 37°C in Micro-aerophilic condition for 72 hours.
Colonial morphology and Biochemicals:
-Produce small, circular, smooth, Non-haemolytic shiny colony.
-H&E for tissue specimens shows small gull wing bacilli.
-Catalase +ve.
-Oxidase +ve.
-Urease Strong +ve.
-Sensitive to cephalithin.
-Urea breath test:
=The patient is given 14C-labeled urea to drink. Urea will be degraded and the patient secrete 14CO2 which is detected by Scintillation counter.
-Rapid Urease test:
=Biopsy is placed on Christensen’s urea broth and incubated for 2 hours at 37°C. Examined for pink colour.
