    هرمون ( HCG) أو ( Human Chorionic Gonadotropin):
    يعتبر تحليل اختبار الحمل ( Pregnancy Test) من أهم وسائل تشخيص الحمل المبكرة وفكرته بسيطة حيث يعتمد على افراز هرمون موجهة القند المشيمائية ( HCG) في بول السيدة الحامل.

    يتزايد هذا الافراز تدريحيآ أثناء الحمل ليصل إلى أقصاه في الاسبوع العاشر ، ثم يعود إلى الهبوط ليصل إلى مستوى ثابت بعد الأسبوع الخامس عشر وإلى إنتهاء الحمل.

    تختلف حساسية هذا الاختبار ، حيث يمكن الشكف عن الحمل بعد 3 أيام من موعد غياب آخر حيض، ولاختبار أقل حساسية يجب أن يمر على الاقل 14 يوم عن موعد غياب آخر دورة شهرية.

    ويراعى عند اختبار الحمل الاتي:
    يفضل البول الصباحي( حيث يكون أكثر تركيزآ) خاصة في الـ 15 يوم الاولى.
    يجب ألا يحتوي البول على بروتين أو دم ( حتى لا يعطي الاختبار نتيجة ايجابية كاذبة).
    يفيد القياس الكمي لهرمون ( HCG) في الحالات التالية:

    متابعة مسار الحمل. في تشخيص حالات الاجهاض ( Abortion) مثل الاجهاض الوشيك ( Imminent Abortion) أو الاجهاض الناقص ( Incomplete Abortion) أو الاجهاض الحتمي ( Inevitable Abortion)، وفي كل الحالات ينخفض مستوى ( HCG) وقد يصبح اختبار الحمل سلبي.
    تشخيص ومتابعة الحمل العنقودي ( Vesicular Mole) ، حيث يرتفع تدريجيآ مستوى ( HCG) إلى مستويات عالية جدآ ( أعلى من مستواه بداية الحمل ) وبعد تفريغ الحمل العنقودي بحوالي 14 يوم يعود إلى المستوى الطبيعي وإذا لم يعد إلى المستوى الطبيعي يجب الشك بظهور ورم مشيمي ( Chorioepithelioma).
    في تشخيص ومتابعة ظهور الورم المشيمي ويدل على ذلك مستويات عالية جدآ من ( HCG) وعودته إلى المستوى الطبيعي دليل الشفاء.
    يرتفع مستوى هرمون ( HCG) في الحالات التالية:

    أورام الخصية(10%).
    التوائم المتعددة.
    ينخفض مستوى هرمون (HCG) في الحالات التالية:

    الاجهاض الحتمي.
    الحمل خارج الرحم.

      TESTING URINE FOR HCG (PREGNANCY TESTING)
      Human chorionic gonadotrophin (hCG) is a glycoprotein
      hormone produced by placental cells soon
      after the fertilized ovum is implanted in the uterine
      wall. It stimulates the secretion of progesterone by
      the ovary. Progesterone maintains the uterus during
      the pregnancy and prevents any further release of
      eggs from the ovary.
      Laboratory pregnancy tests are based on the
      detection of rapidly rising levels of hCG in urine or
      serum. The amount of hormone excreted in the
      urine is almost the same as that found in the blood.
      At about the time of the first menstrual period, hCG
      concentrations in urine and serum are about 100
      mIU/ml and double in concentration every 1–2
      days. Most pregnancies do not require early confirmation
      by the laboratory.
      Medical reasons for requesting a pregnancy test
      Include:
      – investigation of a suspected ectopic pregnancy,
      threatened abortion, or a trophoblastic tumour
      such as hydatidiform mole or choriocarcinoma.
      – checking whether a woman of child-bearing age
      is pregnant before carrying out a medical or
      surgical investigation, X-ray, or drug treatment
      that could be harmful to the embryo.
      In most of these hCG situations a rapid qualitative
      hCG test result is adequate. Occasionally it is important
      to measure the level of hCG e.g. in the
      investigation of trophoblastic tumour when the concentration
      of hCG is high and therefore a
      quantitative test will be required (usually carried out
      in a specialist laboratory).
      Qualitative tests to detect hCG in urine
      Most of the recently developed immunochromatographic
      (IC) pregnancy tests have both high
      sensitivity and specificity. When ectopic pregnancy is
      suspected it is important to use a sensitive test i.e.
      one that is capable of detecting hCG levels below
      100 mIU/ml. Compared with normal pregnancy, the
      hCG level is lower in ectopic pregnancy.
      Most IC card and strip tests are able to detect 50
      mIU/ml or even 25 mIU/ml of hCG. Latex slide
      tests, however, are generally less sensitive, usually
      becoming positive only when the hCG level is over
      500 mIU/ml.
      The specimen is applied to an absorbent pad. The
      antibody-dye conjugate binds to the hCG in the specimen
      forming an hCG antibody-antigen complex. This complex
      migrates by capillary action to the reaction zone where it
      binds to the anti-hCG antibody, producing a coloured band
      (usually pink-rose colour). In a negative test, no coloured
      band is produced. Unbound conjugate binds to the reagent in
      the control zone, producing a coloured band, indicating a correctly
      performed test. A positive pregnancy test is therefore
      shown by coloured bands appearing in both the control and
      test zones . A negative test is shown by a
      coloured band appearing only in the control zone. Most IC
      tests are rapid, providing results within a few minutes

              in case of ectopic pregnancy the number in the BHCG should correlate with the pregnancy as detected by the trasvaginal ultrasound and the last menstrual cycle if the number is less than expected the test should be repeated after 48 hours if there raise in BHCG is less than 60% ectopic pregnancy should be rolled out
