    تعريف

    Osmolality is a count of the number of particles in a fluid sample,expressed in osmols or milliosmols per kilogram of water


    لماذا يطلب؟

    for two reasons:
    to determine whether serum water content deviates widely from normal;
    or screen for the presence of foreign low molecular-weight substances in the blood
    .

    طريقة الحساب


    To calculate plasma osmolarity (osmolality) use the following equation:
    Posm = 2[Na+] + [Glucose]/18 + [ BUN ]/2.8

    The above equation holds true if the glucose & ureas blood chemistry units are given as: mg/dL.

    If the units are given as: mmol/l; then the calculated osmolality is given by:

    P.OSM = 2[NA] + 2[K+] + [Urea] + [Glucose

          thanks but the one that we measure in our lab called serum Osmolality is there a different between them

          another thing we also measure the urine Osmolality which is better than specific gravity The urine osmolality is the best measure of urine concentration with high values indicating maximally concentrated urine and low values very dilute urine. The main factor determining urine concentration is the amount of water which is resorbed in the distal tubules and collecting ducts in response to ADH. In a dehydrated patient with normally functioning pituitary and kidneys, a small volume of highly concentrated urine will be produced. In a patient with fluid overload the opposite will be an appropriate response.

            Well, the serum or plasma Osmolality terms are used interchangeably. I think, when we use the formula which be mentioned above, the term plasma is more convenient while the term serum used in the case of using osmometer .
            There are many applications for osomolality , despite the water is main factor , but we are looking for the presence of foreign substances or evaluation of hyponetremia . This can achieved by calculation what called osmolal gap.
            If a gap > 10 mOsm/kg that indicates of the presence of other compound possibly toxins.
            This test is simple and inexpensive

                    urine osmolality can be done by urine spot or 24 h collection.

                    the question now which it should be arisen,
                    what is your aim for looking urine osmolality

                    depend on your answer , i can tell you the best way

                    in our lab we look at Na in serum first
