    RADIL STANDARD OPERATING PROCEDURE FOR MAKING A BLOOD SMEAR



    Peripheral blood or potassium EDTA anticoagulated blood (1-2 mg EDTA/1 ml blood) may be used. Smears of peripheral blood must be made immediately. Smears made from EDTA-anticoagulated blood should be made within 2 hours of collection. All specimens must be free of clots.



    Step 1. Place a 2-3mm drop of blood about 1cm from the frosted end of a clean slide that is on a flat surface.

    If using a needle and syringe, first remove the needle and then touch the end of the syringe to the slide.







    If using a capillary tube, be sure to fill the capillary at least two-thirds with blood. Touch the end of the capillary to the slide.







    Step 2. With the thumb and forefinger of the right hand, hold the end of a second slide (“spreader”) against the surface of the first slide at an angle of 30-45 degrees.








    Step 3. Draw it back to contact the drop of blood. Allow the blood to spread and fill the angle between the two slides.







    Step 4. Push the “spreader” slide at a moderate speed forward until all of the blood has been spread into a moderately thin film.









    Alternatively, the slide may rest on the forefinger of the left hand. This may make it easier to maintain a moderate and consistent speed when “pushing” the “spreader” slide.









    Note: The film should not cover the entire surface of the slide. In a good film, there is a thick portion and a thin portion and a gradual transition from one to the other. The film should have a smooth, even appearance and be free from ridges, waves, or holes. The end of the smear (the “feathered edge”) should be smooth and even. The edge of the “spreader” must be absolutely smooth. If it is rough, the film has ragged tails containing many leukocytes.






    Shipping containers are available from most laboratory supply companies. It is important that the containers are sealed securely so that the slides won’t fall out during shipping.

      POOR SLIDE QUALITY


      Poor slide quality can seriously impair the microscopist's ability to recognize malaria parasites.


      Below are examples of poor common problems encountered with thick and thin films.






      Examples of poor quality thick film received in reference laboratory, ranging from not enough


      blood, cracked film, poorly dried film, too much blood, etc







      Examples of common faults in the preparation of thin films:
      1. Too much blood: the end of the film is lost and film too thick
      2. Old, devitrified slide or blood clotting when film made
      3. Uneven contact of the spreader or edge of spreader ragged. Too little blood.
      4. Greasy slide
      5. Good thin film
      6. Thin and thick film on same slide

                  Blood Smears
                  Looking at blood smears can be a little tricky. First, this is one of the few situations in which you may need to use your oil immersion lens. Please be careful. It's easy to run it through a slide. Knowing where to look on the
                  slide can make all the difference between being able to see what's there, and frustration.


                  • You want to be way out on what we call the 'feathered edge' of the slide. That is, you want to be in the shallow end of the blood smear, not in the middle of the slide.
                  • You need to find an area where the RBC's just barely touch each other, or are free swimming. If you are looking at an area where the RBC's are piled on top of one another and the WBC's are dark and small, keep going to the thinner end of the slide.

                  • When you study a smear, remember there are three formed elements to assess, plus what can be learned from the background. No matter what you have been told about the disease the slide represents, always look at the
                  • RBC's,
                  • WBC's and
                  • Platelets.
                  The degree of blue staining in the background can also give you an indication of the amount of protein in the blood. The darker blue the background, the more there is. But again, be sure you are looking in the far reaches of the feathered end of the slide.






                  Looking at
                  blood smears can be a little tricky. Be sure you are at the furthest edge where the blood is the thinnest. You want to be where the RBC's are just touching each other.







                  This blood
                  smear shows actively growing yeasts in some of the WBC's. You may need to look at a number of cells to find some, but they are there. This slide is from an AIDS patient and we grew cryptococcus from his blood. He died the same day the sample was taken.[/align]

                        صراحه والله ابداع ما خليتي عليهم اي قاصر يعني اللي ما يعرف يعمل بلد فلم صدقوني بالخطوات والصور يقدر يعلم نفسه
