Diagnosis : primary Hypothyroidism
Summary :
This 50 years old female egypt patient was first seen at our OPD on 25/10/1429. she claimed to be known hypothyroid and on replacement therapy for last one year and which she discontinued herself one month back. She was on Eltroxin 100 mcg once daily.
She complained of generalized weakness, early fatigability, clinically she was euthyroid with no goiter and stable haermodynamically.

Laboratory:

TFT (on10/06/1429): TSH: 17.99 uIu/mL (NR 0.27- 4.2)
FT4 : 8.41 pmol/L (NR 12 – 22)
Thyroid antibodies(ATG,ATPO): positive

Her latest OPD visit was on 15/04/1430 while she was asymptomatic and euthyoid clinically.

Her latest TFT (on 21/03/1430):

TSH: 6.42, FT4: 9.91

She is taking Eltroxin 50 mcg once daily(she reduced dose herself) and during the visit it was advised to increase Eltroxin to 75 mcg once daily. This report is prepared upon request of patient's family
