    Qualitative Sickle cell test was done and reported NEGATIVE for a patient with a Hb 4.5 g/dl with laboratory finding of Hemolytic Anemia .On repetition of the test by another member of staff (With the same sample ) the sickle cell result was reported POSITIVE ..

    because he didnt breakdown the RBC to have the hemoglobin S or he didnt add the chemical needed for reducing oxygen to find out hemoglobin S

        سلام اخي هيماتولوجست ما اادري بالضبط ادا كنت تقصد ال tube test العادي الي اضيف 2مل من الراجنت و 20 ميكرو من الدم
        ادا كان هدا التست ماادري صراحه بس باقول الي اتوقعه والله اعلم والتعليق لكم اخي , في حالات كان الهيموغلوبين منخفض بنسبه عالي اي دون مستوى 7 فهدا التست بعيطيك نتيجه فولز نغيتف ادا بتستخدم نفس الكميه في دي الحاللات كما اعرف يجب ان اندبل الكميه الى 40 ميكرو *ونفس كمية الراجينت 2مل, بالنسبه الى اعادة التست من شخص اخر اتوقع اخد كميه مختلفه عن الاولى فعطاك نتيجة بوسيتف والله اعلم

            in case of low haemoglobin its better to wash the sample and take washed RBC for testing instead of whole blood
            so, it could be due to very low hemoglobin
            or due to technical errors

              الاجابة النموذجية

              The first staff member who performed the test did not follow the instruction which says that in cases of severe Anemia with less than 7g/dl ,the test should be reported by mixing double the amount of blood .The second staff performed the test correct way .

                  What Is Wrong 2

                  G6PD screening was done on a patient with hemolytic anemia having hyperbilirubinemia and reticulocytosis 18 .5 % .The report issued showed NORMAL G6PD activity .Two months later , when the patient visited hospital for follow up , a repeat investigation was done which showed :

                  S.bilirubin : within normal range
                  Reticulocyte : 0.2%
                  G6PD screening : No activity

                  What is the possible explanation for the first report showing false NORMAL G6PD activity .
                  Was there any fault in the technique or was there a problem with the kit ?

                            السلام عليكم
                            there is no technical errors or defaults in the kit but the false normal in the first time due to high reticulocyte count whic is immature RBC and contains high amount of the enzyme
