]الألفا فيتو بروتين(Alpha Fetoprotein) مادة بروتينيه تصنع في الكبد و لكن لا يعرف وظيفته الأساسية.و لقد وجد الأطباء أن قياس هذا البروتين في دم الأم الحامل له فوائد تشخيصية لبعض الأمراض العيوب الخلقية.و الجنين ينتج كميات متفاوتة من هذا البروتين و يمر عبر الحبل السري و المشيمة إلى دم الأم. و لذلك فعند ارتفاع نسبته عند الجنين فان تلك الزيادة أيضا تظهر على شكل ارتفاع في مستوى البروتين في دم الأم.و لقد و جد أن ارتفاعه بشكل ملحوظ قد يعني أن هناك عيبا خلقيا في العمود الفقري للجنين أو بما يعرف بالظهر المشقوق كما انه أيضا يرتفع في حالات و جود ثقب أو فتحة في جدار البطن كما هو الحال في حالة و جود فتق في السره من النوع الذي يكون مصحوب بخروج الأمعاء.كما انه يرتفع في حالة و جود رشح للبروتينات من الكلية او ما يسمى بمرض نفروتك(Finish Type Nephrotic syndrome).
و من المعلوم أن مستوى الألفا فيتو بروتين يتفاوت على حسب عمر الجنين و لذلك فان لكل أسبوع من الحمل مستوى محدد إذا تجاوزه اعتبر مرتفعا.و لذلك فان أو عمل يقوم به الطبيب عند ارتفاع هذا البروتين هو التأكد من عمر الجنين عند اخذ العينية.كما أن مستوى هذا البروتين يرتفع عند و جود توائم, كما أن مستواه يتفاوت إلى حد بسيط حسب و زن الحامل و عمرها و حسب أصولها العرقية.في العادة يجرى هوا التحليل في الأسبوع السادس عشر من الحمل لكل النساء و يمكن إجراءه بعد ذلك التاريخ.
كما أن انخفاض هذا البروتين قد يزيد من احتمال أن يكون الجنين لديه مشكلة في الكروموسومات و بالتحديد مرض متلازمة داون.
إن من المهم معرفة أن تحليل مستوى الألفا فيتو بروتين لا يُعتمد عليه في التشخيص. هو فقد يحدد نسبة احتمال الإصابة.فيقول مثلا أن احتمال الإصابة بمرض ما أعلى من المتوقع أو مساوي للمتوقع.و لذلك فإذا كان التحليل غير طبيعي فانه يلزم الطبيب إجراء فحوصات إضافية للتأكد من سلامة الجنين.فمثلا إذا كان مستوى البروتين عالي فانه يقوم –بعد التأكد من عمر الجنين وقت إجراء الفحص-بالتأكد من عدم و جود توائم و عدم و جود عيب خلقي في العمود الفقري(الظهر المشقوق أو حالة غياب المخ ) أو في جدار البطن و يتم ذلك بالأشعة الصوتية و عند الجمع بين هذا التحليل و الأشعة الصوتية فانه تستطيع أن تكشف الظهر المشقوق بنسبة 95% و تكشف حالة غياب المخ بنسبة 100%. و عند إجراء تحليل الألفا فيتو بروتين فقط فإنها تكشف عن 85% من الحالات فقط.
كذلك الحال عند انخفاض مستوى الألفا فيتو بروتين عن المعدل الطبيعي فانه يجب التأكد من أن الجنين ليس لدية علامات أو أعراض لمتلازمة داون عن طريق إجراء أشعة صوتية للجنين،و قد ينصح الطبيب الأم الحامل بإجراء اختبار للسائل الامينوسي للكشف على كروموسومات الجنين للتأكد بشكل أدق من عدم أو إصابة الجنين بمتلازمة داون أو إدوارد.

How is it used?
The triple or quad screen is used as a screening test in the second trimester of pregnancy. It is ordered to help evaluate the risk that a fetus has certain abnormalities, including Down syndrome (trisomy 21), Edwards syndrome (trisomy 18), and neural tube defects such as spina bifida or a condition called anencephaly.
These tests have been established as a triple or quad screen because their power lies in their use together. A mathematical calculation involving the levels of these 3 or 4 substances (AFP, hCG, unconjugated estriol, and, sometimes, inhibin A) and considerations of maternal age, family history, weight, race, and diabetic status are used to determine a numeric risk for Down syndrome and for a few other chromosomal abnormalities such as Edwards syndrome (trisomy 18) in the fetus. This risk is compared with an established cut-off. If the risk is higher than the cut-off value, then it is considered a positive screening test or increased risk exists for carrying a fetus with one of the discussed abnormalities.
An AFP test may be performed by itself and not as part of a triple or quad screen, especially when First Trimester Screening has already been used to assess the risk for chromosomal abnormalities. The AFP is used to help determine the risk of neural tube defects.
When is it ordered?

What does the test result mean?

A baby with an open neural tube defect has an opening in its spine, head, or abdominal wall that allows higher-than-usual amounts of AFP to pass into the mother's blood. The other markers are not used in the evaluation of risk for carrying a fetus with a neural tube defect.
If a screen is positive, tests that are more definitive are needed to determine and confirm a diagnosis. These include high-resolution ultrasound and perhaps amniocentesis followed by chromosome analysis. These follow-up tests are used to help women and their doctors make decisions about the management of their pregnancies.
The triple screen can detect about 70% of fetuses affected by Down syndrome while the quad screen can detect about 80%. Either screen can detect about 75-80% of those affected with spina bifida and 95% cases of anencephaly.
