well, this patient is indian but no thing to do with bomby blood group
:sm186:I got "oedipus" because of this blood group----------->
as I understood, the blood group was A postive before the cancer complication has take plase ,so the cause of blood group changing is cancer complication
I am thinking if radiation can break down blood group antigen sugers which may change the chractaristic of the blood group from one to another ....:sm198:

Slide method: No agglutination with all anti-sera due to neutrolization of these anti-sera by soluble antigen. In this case patient had soluble A antigen which reacted with anti-sera A and neutrolized it therefore didnt give chance to insoluble A antigen on RBCs to react with it.
As far as Reverse grouping is concerned cancer can effect antibodies production which could give no reaction or very weak reaction
tube method with saline washing 5-8 times should give correct forward blood grouping

what did you meant by neutralization??!! Is it different from the agglutination??!! In such case is there any chance of having
autoagglutination complications??!!....
or maybe because that soluble antigens are free in the plasma not bonded to RBCs ,that is why no auto agglutinatin is taking place

To make you remember and never forget it any more, always translate neutrolization meaning to Stop effect of, so these soluble ABO antigen neutrolize anti-sera effect means stop effect of antisea. Thus when anti-sera effect stopped they are no more able to give their desired reaction with ABO antigen on RBCs.
Antisera still react with soluble antigens but since these antigens are soluble agglutination can not be seen
No chance of autoantibodies because thease soluble ABO antigens are recognised as self and thus no antibodies are produced against it.

A Rh(D) -ve individual who is known to have compatible transfusion previously required one unit of blood again after 5 months. Crossmatch testing with 8 donor units with same ABO Rh phenotype showed incompatibility. Give reasons for that?

أعتقد ان الخطوة الاولى هي عمل" AST "antibody sreening test للمريض وفي حالة انه ها الفحص إيجابي يلزمنا معرفة نوع الجسم المضاد المسبب للتفاعل من خلال استخدام ال panel cell..
بعد معرفة الجسم المضاد الموجود في المريض يتم عمل cell typing للمتبرع للتأكد من عدم احتواء خلاياه على" الانتجين المعروف " المسبب لعدم التوافق...
عندها يسهل علينا اختيار المتبرع المناسب من خلال اعطاء المريض الفصيلة المطابقة لفصيلة ال ABO وخالية من الانتيجين الذي يحمل المريض جسمه المضاد ....

طيب إذا افترضنا إنه المريض لم يتم تكرار عملية نقل الدم عنده يعني ال AST نيجاتيف..
سؤالي هل تم عمل ال فحص الD weak للمريض؟!!
وعندي سؤال ثاني إذا ممكن ؟؟!!
لما نقول انه المتبرعين لهم نفس ال ABO Rh phenotype ,,,هذا ايش معناه؟؟!!
هل معناه انه إذا المريض D partial حتى المتبرع يحمل ايضا نفس الصفة مثلا ؟؟!!

بالنسبة لل AST فهو positive في ال cell I و Cell II
عملية نقل الدم السايقة كانت بنفس فصيلة المريض A RhD negative
المريض مو D weak وحتى لو فرضنا أن المريض D weak نقل دم فصيلة D negative لم يكن ليؤثر فيه لأن D weak يعني D positive ولا يحفز افراز أجسام مضادة ضد ال D antigen
بالنسبة لإحتمال ال partial D فهو مستثنى بسبب أن وحدات الدم المستخدمة جميعها D negative ولو فرضنا احتمالية وجود D partial لندرته في إحدى وحدات الدم لكان Incompatibility لتلك الوحدة فقط وليس لكل الوحدات إذن الحالة تتطلب المزيد من القراءة لتفسيرها.التعديل الأخير تم بواسطة medical lab specialist; الساعة 08-02-2011, 09:33 PM.

طيب بما ان ال "cell l & cell ll " اثنينهم بوستيف ,وال panell cell أصلا مصنوعة من فصيلة ال "O"Rh postive معناها انه الانيجين هو ال "D" لأنه التفاعل صار في الاثنين وأعتقد انه ال "D"هو الانتيجين الوحيد اللي مكرر في ال cell l & cell ll...
دام انه المريض ليس D partial وطبعا ما فيه مشكله مناعية لأنه الفصيلة طالعة A negative ولو فيه أي مشكلة مناعية كان صار تفاعل في كل شي وصارت الفصيلة AB postive..
طيب أحاول أقرا في الموضوع أكثر وان شاء الله نوصل لحل...التعديل الأخير تم بواسطة ambitious553; الساعة 08-02-2011, 10:13 PM.
